LOS ANGELES - Britney Spears' continuing downward spiral took a devastating turn Monday when she was ordered to relinquish custody of her children by a judge who had cited her drug-and-alcohol-fueled lifestyle.

Superior Court Judge Scott M. Gordon ruled that that ex-husband Kevin Federline will take Sean Preston, 2, and Jayden James, 1, beginning Wednesday "until further order of the court."

The order stemmed from an unspecified oral motion made by Federline's lawyers and was handled in a closed-door hearing. The judge's order did not state the reason for the change in custody and he ordered all transcripts of the proceedings sealed.

Phone messages left for representatives of Spears and Federline were not immediately returned Monday.

Last month, Gordon said Spears engaged in "habitual, frequent and continuous use of controlled substances and alcohol" and ordered her to undergo random drug and alcohol testing twice a week as part of her ongoing custody dispute with Federline.

Spears also was previously ordered to meet weekly with a "parenting coach" who was to observe and report back to the court about her parenting skills. Both Spears and Federline must complete the court's "Parenting Without Conflict" class.

Spears, 25, and Federline, 29, were married in October 2004. She filed for divorce last November and it became official in July. The two have joint custody of their sons, but Federline is seeking a greater share of custody.

Since Spears became single, her troubles have played out in the tabloids. First she was photographed at various Hollywood hotspots partying hard with Paris Hilton — appearing drunk and out-of-control. Some photos captured the pop star without panties. Then Spears shaved her head as paparazzi captured the moment, beat a car with an umbrella and ended up spending a month in rehab.

An MTV Video Music Awards performance last month meant to herald her comeback was universally panned, with Spears appearing spaced-out and lethargic — and, in some eyes, less than buff. The following week, Spears' management firm dropped her and her divorce lawyer resigned.

She then was charged with misdemeanor counts of hit-and-run and driving without a valid license for allegedly crashing into a parked car in August.

The driving offenses likely compelled the judge to issue Monday's orders, said New York divorce lawyer Raoul Felder.

"She was driving without a license with a child in the car," he said. "You can put aside the in and out of rehab, the shaving her head. But this? When a judge went out on a limb for her? There's no going back."

Though Spears must relinquish physical custody of the children until further ruling from the court, she and Federline still share legal custody.

Los Angeles divorce lawyer Connolly Oyler said he's surprised the children weren't taken from Spears sooner. Judges typically remove youngsters from households where parents are ordered to undergo drug testing, he said.

"There may have been another incident and the judge had had enough," Oyler said.

But, he added, all is not lost for Spears.

"If she picks a good counselor and gets a different set of friends, there's a good chance she can turn things around," he said, noting "she's only 25."

The couple is due back in court Oct. 3. Spears' new album is set to be released Nov. 13.

Back home, Britney Spears spent the early part of Saturday with dad Jamie after being released from Cedars-Sinai hospital, PEOPLE has confirmed.

Spears's mom Lynne, who yesterday called for prayers for the embattled singer, remains in their hometown of Kentwood, La.

Spears left the Los Angeles medical facility Saturday via an underground tunnel and was accompanied by TV therapist Dr. Phil, according to etonline.com, who first reported the news.

A Cedars-Sinai spokesperson declined to comment, but Dr. Phil released a statement to ET saying: "My meeting with Britney and some family members this morning in her room at Cedars leaves me convinced more than ever that she is in dire need of both medical and psychological intervention.

"She was released moments before my arrival and was packing when I entered the room. We visited for about an hour before I walked with her to her car."

Spears had been committed early Friday morning under California's "5150 Hold," in which Spears was deemed a danger to herself and could be held for 72 hours. Patients however can be released early if doctors determine they are no longer a threat to themselves.

"The 5150 legality is very loose," says Dr. Nathan Kuemmerle, a West Hollywood psychiatrist. "The 72 hours are not firm. All it requires is an ER psychiatrist to release the patient. ... If Britney isn't suicidal or homicidal, she doesn't need to be hospitalized."

Troubled Britney Spears continued her bizarre behaviour as she decided to go out in her old wedding dress with her new boyfriend.

The strange fashion choice has sparked rumours that her new man, photographer Adnan Ghalib has concocted a plan to marry the star in an attempt to secure her fortune and lucrative exclusive picture deals.

Britney is apparently also convinced a new marriage will help her win back her children who are in the custody of her ex Kevin Federline.

Seen out wearing the tiny, white lace wedding dress she wore for wedding number two with K-Fed, Britney walked side-by-side with her new man.
Britney gave the dress its first outing at her wedding reception with father of her two sons.

She had been married once before to childhood friend Jason Alexander but the union was quickly annulled.

After a whirlwind two-week romance with Ghalib, the pop star has apparently told friends she wants to marry the photographer and convert to Islam.

The twice-married singer, 26, has been in hiding in Los Angeles with Ghalib since her public meltdown last week which saw her lose custody of her two sons to ex Kevin Federline.

It was claimed that during a phone call to a relative, the photographer said he had deep feelings for the star, insisting they had discussed marriage.

Britney also apparently chipped during the phone call and said: "We're taking care of each other. He's the only one who understands me. It's serious."

The mixed-up pop princess then extended an invitation to her lover's family to visit them in the US when she has got over her problems. The biggest of which is probably tomorrow's custody hearing which will decide whether she will be able to see children Sean Preston, two, and one-year-old Jayden James.

Last night a source close to Britney said: "Monday's court hearing is D-Day for her and the kids. While she's been swanning around with Adnan, Kevin's been preparing for the court showdown to end them all. He has no confidence in her ability to look after either herself or the children."

However, a source close to Birmingham-born photographer painted a different picture and revealed how Ghalib has boasted to friends about their wild relationship and drug-fuelled romps.

"He said he'd 'struck gold' in all ways and was going to keep hold of her for as long as he could."

One of his relatives has also stepped in and commented on the relationship.

"All he wanted to do was get close enough for some special pictures.

"But now he daren't lift a camera when he's with heror she'll rumble him and kick him out.

"So instead he's cleverly changed tack, says he's planning a long-term relationship with Britney and that they have discussed marriage.

"He is still wed so he knows he can't do anything until there's a divorce from his wife. He's probably working on that already."

A close friend of Britney revealed how fragile the star is.

"She's been discussing all these wacky plans to reinvent her life and convince the courts she is a good mother. She has discussed in depth a fake death, moving abroad and even plastic surgery. It is scary to hear her romanticise about these insane plans.

"She believes she could spend six months away and make a comeback as Britney the world's best mother."

Her Psychiatrist Gave the Order

Once again, Spears was committed using California's 5150 emergency hold, but this time, a source says, it was her psychiatrist – Santa Monica-based Dr. Deborah Nadel, who is certified by the American Board of Psychiatry and Neurology – not police, who issued the order.

"This means it might be a more favorable outcome," says Dr. Diana ***schner, a New York City psychologist who has not treated Spears. With mental illness such as bipolar disorder – which two sources tell PEOPLE Spears has been diagnosed with – patients "have to make a connection [with a mental health professional] to move forward," says ***schner. "There is a way you can bond with a client even though you are slapping them with something like [the 5150]."

Although pal Sam Lutfi told PEOPLE that Spears entered the hospital "willingly," Dr. Mark Goulston, a Santa Monica psychiatrist, points out that the 5150 is by necessity "an involuntary hold. ... It may be that she went in willingly, [but] her mental state was seen to be so volatile that whomever evaluated her didn't trust it would change."


Her Severe Lack of Sleep
Sleeplessness – friends say Spears hadn't slept since Saturday – "is a cardinal symptom of bipolar disorder," says Dr. Goulston, who has not treated Spears. "Not sleeping for three days could interfere with all of us functioning at a reasonable level. In someone who has bipolar disorder, sleep deprivation can trigger a full-blown episode. One of the things they will want to do in the hospital is get her to sleep. The question is: When she wakes up, will she be sane enough to leave, and then not accept treatment? That will be the challenge."


Possibility of Longer Hospitalization
Although the 5150 typically involves a 72-hour hospitalization for treatment and evaluation, when she was last hospitalized Spears left Cedars-Sinai Medical Center after just two nights. Currently, "[Nadel] has the power to decide whether Spears can be released in that time frame or not," says a veteran L.A. psychiatric social worker. "However, [Nadel] is only one opinion. Within the three-day hold, patients-rights workers from the L.A. County Department of Mental Health will interview Spears and review her charts to determine whether the hold is legally justified." After 72 hours, "[Nadel] can request a 14-additional-day hold called a 5250."


Medication
Although Spears is more likely to comply with medical orders in a hospital setting, forcing her to take medication can still be challenging. "If she refuses to take meds that a doctor determines she needs, [Nadel] can request a court to grant that power – but that's a lengthy process that won't happen in just 72 hours," says the social worker. "I've seen patients housed for 40 days with not a single pill given."


Family Involvement
Although her dad Jamie visited Britney in the hospital during her Jan. 3 hospitalization, her mom Lynne remained in Kentwood. This time, Lynne has been present throughout the crisis. Still, "during a 5150 hold, a patient's family has no power unless the patient is a minor," says the social worker. "This is because, in part, family involvement could create further conflict. Even visitation can't be granted to family unless the patient asks for it in writing."

Says Dr. ***schner: "The fact that the person's family is around them is generally favorable." Given the fact that she was hospitalized a second time under a 5150, "she was probably increasingly worrying people who were worried for her well-being," says Dr. Goulston. "If she's driving and she's actively bipolar she is a danger to herself. The question is: How many of these will it take to stick and for Britney to be willing to get help?"

Attorneys for Britney Spears and her father attended a closed-door emergency hearing Thursday, a day after the singer's early release from a psychiatric hospital.

The attorneys met before Commissioner Reva Goetz, who recently extended temporary control of Spears's affairs to her father Jamie and a lawyer. The session lasted about a half hour, and the lawyers left without commenting.

Apearing in Goetz's downtown L.A. court were Jeryll S. Cohen and Vivian Thoreen, who represent Jamie; Samuel Ingham, a court-appointed lawyer for Britney; and Andrew Wallet who, along with Jamie, is co-temporary conservator of the pop star's estate.

Adam Streisand, an attorney (and relative of Barbra Streisand) who had once claimed to represent Spears – and who met with her Wednesday night – was not there; his firm announced he was off the case.

Attorneys for Britney Spears return to court Tuesday morning for the first scheduled custody hearing since the pop star's most recent hospitalization.

Insiders doubt there will be any major change from the current custody arrangement, which bars Spears from visiting her sons Preston, 2, and Jayden, 1. (She is, however, allowed telephone contact.) Still, it's possible the 26-year-old singer – who hasn't seen the boys since her Jan. 3 standoff with police – could be awarded some temporary physical access.

Tuesday's court session – which was scheduled after Spears's stay in the psych ward at the UCLA Medical Center – sets the stage for more major hearings in the coming weeks, as Spears and Kevin Federline seek to reevaluate their custody agreement. (Her former husband and backup dancer is seeking full physical custody.)

The judge may also hear arguments from Federline's attorney, Mark Vincent Kaplan, who has asked for a full psychological evaluation of the troubled pop star.

But the biggest news out of today's hearing could be a change in representation for Spears. Los Angeles attorney Sorrell Trope has asked – for the second time – to be released from the case. (The law firm of Trope & Trope filed similar papers in January, but later withdrew the request.

Possible replacements include Laura Wasser, who resigned from Spears's case in September. At the time, she denied "dumping" Spears, telling PEOPLE the singer's legal team simply needed "fresh blood." But Wasser appears to be back in the picture, after reportedly visiting Spears during her UCLA stay.

UCLA Medical Center will be firing at least 13 employees and is disciplining others, including physicians, for looking at Britney Spears's medical files, the Los Angeles Times reports.

Spears, 26, was involuntary hospitalized at the campus's psych ward for one week beginning Jan. 31, after her doctor made the request.

But the records reviewed by those who are targeted for dismissal were from previous hospitalizations at UCLA, the Times reports. The dates and purposes of those hospitalizations weren't known.

UCLA officials declined to comment on the reported firings, but said in a statement: "UCLA Health System considers patient confidentiality a critical part of our mission of teaching, research and patient care. ... We have stringent policies to protect patient confidentiality and address violations of those policies."

Britney Spears has received a third nomination for the MTV Video Music Awards as speculation mounts about her appearing at the event following her calamitous comeback last year.

Nominations for various categories are slowly being announced by MTV, with those for Video of the Year and Best Rock Video announced on Friday.


Spears was named as one of five candidates for the Video of the Year award, for her single 'Piece of Me'. She has already been nominated for the same video clip in the Best Female Video and Best Pop Video categories.


The US singer will face competition in the Video of the Year category from Jonas Brothers' 'Burnin' Up', Chris Brown's 'Forever', Pussycat Dolls' 'When I Grow Up' and the Ting Tings' 'Shut Up and Let Me Go'.


Meanwhile, Paramore's 'CrushCrushCrush', Slipknot's 'Psychosocial', Linkin Park's 'Shadow of the Day', Foo Fighters' 'The Pretender' and Fall Out Boy's cover of Michael Jackson's 'Beat It' will fight it out in the Best Rock Video category.


Other categories announced so far include Best Male Video, Best Hip Hop Video, Best New Artist and Best Dancing in a Video.


The 25th MTV Video Music Awards will take place in Los Angeles on 7 September, though neither the broadcaster or Spears has yet confirmed whether the star will again use the live show as a platform for another comeback.


Last year Spears gave an ill-fated and critically-mauled performance of her song 'Gimme More'.


The star has appeared in a number of recent promotional videos for the upcoming awards show, which will be hosted by British comedian Russell Brand.
