FDA approves Abiomed's heart pump Impella, shares rise

Medical device maker Abiomed Inc said U.S. health regulators approved the sale of its heart pump, Impella 2.5, driving its shares up about 24 percent to their highest in more than six years.
Impella 2.5, which Abiomed said is the world's smallest heart pump, is approved in the United States for partial circulatory support in critical patients for up to six hours.
The approval by the U.S. Food and Drug Administration allows Abiomed to begin selling the device to an estimated 14,000 interventional cardiologists at about 1,700 heart hospitals in the United States.
The company said the device is now approved in more than 40 countries and has been used to treat more than 1,500 patients outside the United States.
Shares of the Danvers, Massachusetts-based company were trading up $1.96 at $16.74 in midday trade on Nasdaq. They touched a high of $18.28 earlier in the session.

J&J Wins U.S. Panel's Backing for New Psoriasis Drug
By Catherine Larkin and Alex Nussbaum

June 17 (Bloomberg) -- Johnson & Johnson won a U.S. panel's unanimous backing to introduce a new drug for millions of adults with the skin disorder psoriasis.

Outside advisers to the Food and Drug Administration recommended approval of the injectable medicine, ustekinumab, in an 11-0 vote today in Silver Spring, Maryland. The FDA usually follows its panels' advice, though it isn't required to do so. Ustekinumab would compete with Enbrel, sold by Amgen Inc. and Wyeth; Humira from Abbott Laboratories; and J&J's own Remicade.

As many as 7.5 million Americans have psoriasis, according to the National Institutes of Health, and ustekinumab would offer less-frequent dosing than existing therapies. While the panelists want more data on a potential link to increased cancer risk, they agreed the drug worked and should be made available to patients while J&J completes long-term safety studies.

``We know it can help them,'' said panel member Lynn Drake, a dermatologist at Massachusetts General Hospital in Boston. ``We don't know if it can hurt them. The answer is to make sure there are proper follow-up studies. Ethically, we have to make this available to our patients.''

J&J, of New Brunswick, New Jersey, fell 25 cents to $65.59 at 4:15 p.m. in New York Stock Exchange composite trading before the panel voted. Amgen, based in Thousand Oaks, California, rose 40 cents to $44.39 in Nasdaq Stock Market composite trading. Wyeth, of Madison, New Jersey, gained $2.08, or 4.8 percent, to $45.16, and Abbott, of Abbott Park, Illinois, fell 76 cents, or 1.4 percent, to $53.57 on the New York Stock Exchange.

                Enbrel's Dominance

Psoriasis occurs when the immune system attacks healthy cells, causing patches of red, scaly skin on the scalp, joints or other parts of the body. Enbrel, Humira and Remicade treat the disease by soaking up excess amounts of TNF, an inflammatory protein. Enbrel dominates as much as 80 percent of the psoriasis market, according to a June 13 note from Cowen & Co.

The FDA's staff questioned in a review posted this week whether ustekinumab poses a higher cancer risk than other immune- suppressing drugs for psoriasis because of the proteins it targets.

``The data presented suggested some side effects,'' said Susan Walker, the FDA's director of the Division of Dermatology and Dental Products, after today's meeting. ``The FDA will continue to review the application and take the committee's advice into account.''

                Targets Proteins

The drug blocks interleukin-12 and interleukin-23, two proteins that help regulate the immune system and are thought to play a role in inflammatory disorders. Animal studies suggest that interleukin-12 also plays a role in fighting tumors.

Ustekinumab has been studied in 373 patients for at least 18 months, not enough to assure its safety for a chronic condition, panel members said today.

``They haven't told us how long it is safe to use this drug by any stretch of my imagination,'' said panel member Robert Stern, chairman of dermatology at Beth Israel Deaconess Medical Center in Boston.

J&J proposed a voluntary registry of all patients prescribed the drug and to continue following participants in its clinical studies for an additional five years. The panel recommended that doctors administer ustekinumab in their offices instead of patients taking the drug at home, like Enbrel, to avoid misuse and help patient monitoring.

                Fewer Doses

After an initial boosting phase, ustekinumab would be given once every three months. Patients must take Remicade every eight weeks, Humira every other week and Enbrel twice a week. J&J proposed two dose strengths of ustekinumab based on a patient's weight.

Bernadette Dougherty, a Waterloo, Illinois woman who took ustekinumab during a J&J-sponsored study, said she and other people with psoriasis are desperate for a better treatment.

``If we could inject once every three months and then go on, we could be almost like human people,'' Dougherty, 44, told the panel. ``We wouldn't have to get all the questions all the time about, `What is it? Is it contagious?' This drug has to be passed.''

Sales may reach $500 million by 2011 if the FDA approves ustekinumab by its scheduled action date on Sept. 29, JPMorgan analyst Michael Weinstein said in a June 13 note to clients. Sales may exceed that if a study to be released in September shows ustekinumab works better than Enbrel, which garnered about $770 million as a psoriasis drug last year, Weinstein said.

Enbrel, Humira and Remicade also are approved to treat arthritis. Enbrel was the world's fifth-biggest drug last year with $5.28 billion in sales. Remicade brought in $5.21 billion and Humira had sales of $3.06 billion.

Salmonella Investigators Check Causes Beyond Tomatoes

                  By Justin Blum

June 27 (Bloomberg) -- The U.S. is examining whether something other than tomatoes may be the source of a salmonella outbreak that has now sickened 810 people.

While tomatoes remain the likely cause for the growing toll of illnesses reported, investigators can't rule out that another food is to blame, said Patricia Griffin, of the U.S. Centers for Disease Control and Prevention in Atlanta, on a conference call today with reporters.

``There's a strong epidemiologic association with tomatoes,'' Griffin said. ``We have also kept an open mind about other possibilities and are looking into other ingredients. And when we know anything we will certainly let people know.''

The number of people who have become ill is a record for salmonella cases associated with produce, according to the CDC, and is about four times the 205 cases reported after packaged fresh spinach was tainted by E. coli in 2006. The tally of reported illnesses described by the CDC increased by 54 since yesterday alone.

The outbreak began in April, and the most recent case involved a person who said they became ill on June 15, according to the CDC. The outbreak, linked to illnesses in 36 states and the District of Columbia, is ongoing and the CDC doesn't know whether the number of cases has peaked, Griffin said. There is often a lag of more than two weeks before cases are confirmed.

                  Store Shelves

Investigators can't rule out that tainted products are still on store shelves or that tomatoes are continuing to be contaminated in a packing house or some other point in the distribution chain, said David Acheson, the Food and Drug Administration's associate commissioner for foods, during the conference call.

FDA officials have said they believe the outbreak involves tomatoes grown in Mexico or Florida, and investigators are checking points along the distribution chain leading from those locations to pinpoint the source of contamination. Samples of tomatoes taken during the investigation have all tested negative, Acheson said.

Acheson said FDA may never find the cause of the outbreak, repeating a possibility he raised previously. The task becomes more difficult as the trail grows ``cold,'' he said.

``It's possible that this investigation will not ultimately provide a smoking gun that enables us to pinpoint the source of the contamination,'' Acheson said.

The FDA is continuing to check ``up and down the chain'' trying to find the source, he said. Investigators have identified ``clusters'' of illnesses from certain regions and are trying to use those cases to trace the cause, according to the FDA.

                  Packaged Together

Tomatoes have been shipped from Mexico to Florida, combined with Florida tomatoes, and packaged together, complicating the FDA's investigation, Acheson said. In other cases, tomatoes were shipped from Florida to Mexico for packaging, and then shipped back to the U.S.

Cherry, grape, vine and home-grown tomatoes from all regions remain safe to eat, according to the FDA. Before buying raw red plum, Roma or round tomatoes, consumers should ask where the products were grown, according to the FDA.

If the tomatoes are from locations listed as cleared on the FDA's Web site, the products are safe to eat. If they aren't on the list or vendors don't know where they're from, the tomatoes shouldn't be consumed, the FDA says.

Tomatoes from states that have been cleared could still be contaminated in a warehouse or another point in the distribution chain, Acheson said.

Most people infected with salmonella have diarrhea, fever and cramping for four to seven days and recover without treatment. Infants, the elderly and people with impaired immune systems may develop severe cases that lead to hospitalization or death.

Self-Burning: a Common and Tragic Way of Suicide in Fars Province, Iran

                            Abstract:
Self-burning is the most devastating burn injury. It is a common social and medical problem in Iran. In a longitudinal prospective study, from April 2003 to March 2006, all burn patients admitted to Ghotb-eddin burn Hospital were enrolled in this study. Suicide attempts by burning accounted for 283 (21.9%) of all burn patients admitted to the hospital. Most (68.2%) of self-burning patients were female. Self-burn patients had significantly large burned body surface area (64.8±29%). Suicidal burns occurred predominantly in the age group 15-24 years (44.6%). The mortality rate for suicidal burns was (60.4%). Fars is a province composed of many different tribes and cultural minorities whereby major decisions are made by forums of clergymen. It is important to involve those clergymen in burn prevention programs to achieve a meaningful reduction in Self-burning prevalence.

Has the time come for a third class of drugs?

If a behind-the-counter category of drugs is approved, pharmacists can reap professional and financial benefits from serving as the gatekeepers.

Imagine this scenario. You hold a cholesterol screening clinic at your pharmacy. In walks a middle-aged woman who couldn't go to a physician because she doesn't have insurance. You perform a blood test and find that her cholesterol level is dangerously high. You explain the benefits of taking cholesterol-lowering medication and making lifestyle changes. You prescribe a behind-the-counter (BTC) cholesterol-lowering drug, advise the patient how to take it, and discuss potential side effects and drug interactions. You charge the patient for the screening, consultation, and the medicine. After the consult, the patient makes an appointment to return for a follow-up so that you can monitor her drug therapy.

This scenario may be played out in pharmacies nationwide if the Food & Drug Administration approves a third, or behind-the-counter (BTC), class of drugs. Once opposed, the FDA is now exploring the idea of making certain drugs available BTC at the pharmacy without a prescription, but only after intervention of a pharmacist before dispensing. FDA posted an announcement in the October 4, 2007, Federal Register; then held a public meeting on November 14, 2007, to elicit comments.

Elevating the role of the R.Ph.

Steve Giroux, the National Community Pharmacists Association president, envisions that a BTC class would increase patient access to prescription medications and improve patient safety. "If the category creates an opportunity for drugs that are currently restricted to Rx-only sales and if we can get consumers more access and quicker access to potentially better products, it makes a lot of sense," said Giroux.

Michael D. Hogue, Pharm.D., assistant professor of pharmacy practice at the McWhorter School of Pharmacy Samford University, echoed Giroux's sentiments about the benefit to patients of increased access. "There are far too many people who are uninsured or underinsured who simply can't afford to seek care from a physician on a regular basis. I don't think a BTC category exempts a patient from having to see their physician, but it offers us an opportunity to reduce the number of physician office visits that are necessary and hopefully to remove that access barrier to the pharmaceuticals."

William Zellmer, deputy executive VP of the American Society of Health-System Pharmacists (ASHP), anticipates that a BTC class would elevate pharmacists' clinical role and improve patients' health. He argued that by making certain prescription medications more widely accessible, public health issues such as hypertension and hypercholesterolemia may improve. "Because these drugs are available by prescription only from a physician, there seems to be a certain segment of the population who are not using these medications, and yet there are some safety concerns surrounding the medication which makes us hesitant to make them OTC. The idea would be to create an intermediate class of medications, where the prospect for making them OTC could be evaluated, and while this evaluation is going on, these medications would be available from pharmacists," Zellmer explained.

ASHP has recommended to FDA that the agency launch some demonstration projects around the country to assess a couple of models, including payment for pharmacist services. "If that were done, we would strongly advocate that outpatient pharmacy departments in hospitals be part of those experiments, because in many respects pharmacies in an institutional environment are used to practicing in this environment," said Zellmer.

                              BTC ruffles some feathers

While many associations applaud the FDA's desire to study the public health implications of making certain drugs available BTC, several groups are adamantly opposed or have some serious concerns about it.

In a statement to FDA, Joseph W. Cranston, Ph.D., the American Medical Association's director of science, research, and technology, said, "The AMA supports the present classification of drugs as prescription and OTC and opposes the establishment of a new, BTC category. When a drug product is considered unsafe without supervision, a physician should be responsible for supervising the use of that drug. There is little scientific evidence to support the need for a third class of drugs, and many questions about the impact of this class remain unanswered. The AMA is concerned that a third class of drugs could have a negative impact on patient access to some drugs, reflect an increase in costs, and potentially harm patients. This has the potential to put patient health in jeopardy by taking a vital step out of physician diagnosis and treatment of patients."

                                CONTINUED:

The Consumer Healthcare Products Association (CHPA) is also opposed to a BTC class. David Spangler, CHPA senior VP of policy and international affairs, asserted, "We believe that the existing system works well for consumers; it gives consumers choice and convenience in a competitive environment that keeps costs down. The existing system has enough flexibility in it so if there are specific challenges to a specific (drug) switch, sponsors can come up with the tools to address that. You don't need a change in the law to expand access and you don't need to create a new class of drugs. You don't need to give FDA additional authority. The negative ramification if you are creating a class is that you are not empowering consumers to take control. We believe very much that consumers should be in the driver's seat. That's the whole purpose of having medications nonprescription."

While not taking a position for or against a BTC class, the National Association of Chain Drug Stores is concerned that if the class were created, pharmacists should not be forced to take on a law enforcement role. Kevin Nicholson, R.Ph., J.D., VP of pharmacy regulatory affairs at NACDS, said, "We would defer to FDA to determine if the class should be created and what drugs would be included in that class. We recently saw pseudoephedrine (PSE) products become basically behind the prescription counter, even though they are available in other retail outlets. We have some concern that maybe people are thinking that since PSE has gone BTC, maybe we should put other nonprescription products BTC for enforcement purposes. We would rather have the discussion surround the clinical services that a pharmacist can provide."

While the Food Marketing Institute (FMI) applauded FDA's decision to study the concept, the association, which represents supermarket pharmacies, has similar concerns. Cathy Polley, R.Ph., FMI's VP, pharmacy services, said, "The BTC class should not be used as a means to enforce age limits, quantities of product purchased, or other drug enforcement concerns. Rather, creating this class should benefit consumers by making vital medicines available to them—dispensed judiciously by pharmacists—that previously could be obtained only with a prescription."

                                BTC drug candidates

One of the crucial questions that must be asked if a BTC class is created is which drugs are suitable candidates? Zellmer believes that initially a BTC category wouldn't have a large number of medications. He foresees certain statins, some antihypertensive medications, and the oral flu vaccine as possible BTC candidates. Hogue cautioned that decisions about which drugs should fall under this category need to be made on an individual basis. He gave the example of statins, noting that Crestor (rosuvastatin, AstraZeneca) requires renal and hepatic monitoring. He said that pharmacists may not be capable of conducting this monitoring until more point-of-care testing devices are available that allow pharmacists to monitor these more complex therapies.

Hogue also cautioned that certain antihypertensives that affect electrolytes may also not be suitable for BTC because of the level of monitoring and management that is needed. He also advised against moving heart failure and epilepsy drugs BTC.

Hogue has proposed to the FDA that vaccines be the first drugs to move BTC. "We can increase access to a preventive therapy that we know works and know can save lives. Pharmacists have proven over the last 12 years to be extremely capable of identifying patients in need of vaccinations and immunizing them appropriately. There is absolutely no reason a patient needs to go to a physician to be vaccinated against a condition," he said.

Hogue also feels confident that nasal corticosteroids and nicotine replacement therapies are suitable BTC candidates. "The management of allergic rhinitis is very straightforward, easy to diagnose, and pharmacists can easily determine dose and identify which corticosteroid to use. Nicotine replacement therapy and other agents used in the treatment of smoking cessation are also excellent drugs to go BTC, said Hogue.

Melissa Sherer, Pharm.D., a consultant for Pharmacy Healthcare Solutions, agreed that smoking cessation products, when appropriately monitored, would be effective BTC drugs. Sherer cautioned, though, that pharmacists might need to verify that the patient is over the age of 18 before prescribing these drugs. "We'd probably see a similar age requirement on those types of products that would be more like Plan B in terms of age checking," she said.
